Remarks
Reading returns the custom width, DefaultColumnWidth when none is set, or 0 when the column is hidden (use HiddenColumnWidths for the width to restore).
Writing stores a custom width (negative values are clamped to 0); assigning the current DefaultColumnWidth removes the custom width. Out-of-range indexes are ignored. Each assignment recalculates the layout unless an update block is active, so wrap bulk changes in BeginUpdate/EndUpdate.
